Privacy Act Release Form
The Privacy Act Release Form must be completed whenever you request information from your file, even if the information is being released to you. The form must also be completed when you are asking NHSC to verify the amount of your scholarship support to a third party. Before information or documentation can be released, the form must be returned with an original signature (no faxed copies).
